GLP-1 receptor agonists like semaglutide (Ozempic, Wegovy) and tirzepatide (Zepbound, Mounjaro) have revolutionized weight management and diabetes care, offering significant benefits in blood sugar control, appetite suppression, and weight loss. However, as their popularity grows, so does the need to understand their potential risks—one of which is osteopenia (low bone mineral density), a precursor to osteoporosis and increased fracture risk.


Why Do GLP-1 Medications Increase the Risk of Osteopenia?


  1. Rapid Weight Loss and Bone Density: Significant and rapid weight loss, a common effect of GLP-1 medications, can reduce mechanical loading on bones, leading to decreased bone density. Studies suggest that weight loss of more than 10% can result in a measurable reduction in bone mineral density (BMD)—especially in postmenopausal women, a population already at risk for osteoporosis.


  2. Potential Hormonal Changes: GLP-1 medications influence multiple hormonal pathways, including insulin, glucagon, and even parathyroid hormone (PTH), which regulates calcium and bone metabolism.
